Ontario medical forms for physicians, NPs and allied health

Every verified Ontario form in one list, from WSIB and FSRA auto forms to ODSP, OSAP, ADP and MTO, with who completes each one and a link to the official source.

Ontario has the largest section of this library, with 150 verified forms. The WSIB accounts for 42 of them, led by the Health Professional's Report (Form 8, 0008A) after a first visit and the Form 26 progress report. Auto accident benefits run on FSRA's OCF forms, such as the OCF-3 Disability Certificate and the OCF-18 Treatment and Assessment Plan. ODSP, OSAP, Assistive Devices Program funding, Ontario Drug Benefit exception requests and MTO driver reports round out the list.

Form 8 sent electronically through TELUS Health earns a higher WSIB fee than paper, and occupational mental stress claims use Form CMS8 instead. The OCF-18 and OCF-23 go through HCAI at registered facilities, while the OCF-3 goes to the insurer. Many ministry e-forms, including the ADP device applications, are XFA files that fill correctly only in Adobe Acrobat or Reader. Most forms here also come in French. Federal forms such as the T2201, CPP disability and EI certificates apply in Ontario too and are on the federal hub.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Is there a downloadable ODSP Health Status Report?

No. The Health Status Report and Activities of Daily Living Index come in the paper Disability Determination Package that ODSP issues to the applicant, or you complete them online in SADIE with a GO Secure account, your OHIP billing number and the patient's Member ID and Referral ID. The patient has 90 days to submit. Physicians bill OHIP, and other professionals invoice MCCSS on form 3261E.

Who pays for completing the OCF-3 Disability Certificate?

The auto insurer. The OCF-3 states that the completion fee must be billed to the insurer directly. A health practitioner as defined in the SABS completes Parts 5 to 10, covering injuries with ICD-10-CA codes, disability tests, investigations, prior conditions and medications, after the applicant fills Parts 1 to 4. Return it to the applicant's auto insurer; it isn't on HCAI's list of provider forms.

Why won't some Ontario government forms open in my browser?

Many forms from Ontario's Central Forms Repository are XFA-based e-forms, including the ADP device applications, the MTO Commercial Driver Medical Report (SR-LC-080E) and the Medical Certificate of Death (11291E). They open and fill correctly only in Adobe Acrobat or Reader, because browsers and most PDF libraries cannot show their fields. Save the file and open it in Adobe Reader instead.

When do I use the MTO Medical Condition Report (5108E)?

Use it to tell the Registrar of Motor Vehicles about a patient with a prescribed high-risk condition, which is a mandatory report, or another condition that may make driving dangerous, which is discretionary. Physicians and NPs can file either type, OTs file discretionary reports only, and optometrists use the Vision Report (5109E). Physicians and NPs at participating clinics can submit through eForms. A ticked mandatory condition results in licence suspension.
