WSIB mTBI Program of Care Mid-Point Report (10392A)

At the end of block one of the WSIB mild traumatic brain injury program of care, the treating provider files this two-page mid-point report. It compares Patient-Specific Functional Scale scores from the initial assessment with current scores, documents vestibular rehabilitation delivered or planned for block two, and records any recommended referrals, such as the WSIB Community Mental Health Program or the Neurology Specialty Program.

Who completes it

The registered regulated health professional delivering the program, such as a chiropractor, occupational therapist or physiotherapist, completes and signs the report.

Submitting and payment

How to submit it
Submit it through wsib.ca/submit within two business days of the last block one treatment; a report the WSIB has not received within 14 days is not paid.
Who pays
Bill the report under MTBRMPR, the service code printed on the form; block two is billed under MTBRTB2, or MTBRTB2V with vestibular rehabilitation, and needs at least four visits for the full fee.

Good to know

  • Block payments are withheld until the WSIB receives the corresponding report, and the report fee is lost if the report is not received within 14 days.
  • Record PSFS scores for the same three to five activities, at least two work-related, with the initial and mid-point scores and your assessment of current ability.
  • If vestibular rehabilitation is being provided or recommended for block two, give objective findings, the rationale and the delivered or planned interventions.
  • Referral options include psychology through the WSIB Community Mental Health Program, psychiatry, an OHAP mTBI assessment, the Neurology Specialty Program and a WSIB return-to-work specialist.
