WSIB Form 8 (0008A): Health Professional's Report

The WSIB expects this report after your first visit with a patient whose physical injury or illness is, or may be, work-related, even if they were first seen in an emergency department. It gives the WSIB your findings, diagnosis, treatment plan and return-to-work advice. Page 2 reaches the employer through the patient, while the WSIB scans the full report into the claim record and sends it for payment.

Who completes it

Physicians, nurse practitioners (RN Extended Class), chiropractors and physiotherapists complete Sections B to F and sign the return-to-work page. The patient completes Section A (patient and employer information) and signs Section G to authorize sending page 2 to their employer.

How to fill it in

A. Patient and Employer Information (patient to complete)

The patient fills in their name, address, telephone, Social Insurance Number, date of birth, sex, preferred language and employer name, plus the claim number at the top if they know it. Review it before you send the report. WSIB says Form 8 payments are delayed when Section A is incomplete, illegible or incorrect, so make sure every identifier is filled in and readable.

B. Incident Dates and Details Section

Describe how the injury, reinjury or illness occurred at work, and record the patient's occupation and the date of the incident or when symptoms started. Use the form whether the patient says the condition is work-related or you believe workplace factors caused it. The worker and employer report the injury to the WSIB separately; your obligation is to provide information about the worker's health care.

C. Clinical Information Section

Check all that apply. Mark each area of injury or illness, with right or left where the form asks. Under physical examination findings, tick items such as fracture, sprain/strain, laceration or reduced range of motion, note pain at rest or night pain and a 0 to 10 pain rating, and mark exposures such as fumes, needle stick, hearing loss or hand-arm vibration. Then note any pre-existing or other conditions that may affect recovery, and record your diagnosis.

D. Treatment Plan

Describe the treatment plan, including type of treatment, duration and prescribed medications. Item 2 is for physicians only: list up to four work injury or illness medications with dose, frequency and duration. Item 3 records investigations (labs, X-rays, CT, MRI, EMG, ultrasound) and referrals the patient would benefit from, such as physiotherapy, psychology, a specialist, an occupational health centre or a Regional Evaluation Centre, with the referral name and appointment date if known.

E. Billing Section

Tick your designation (chiropractor, physician, physiotherapist or registered nurse in the extended class) and enter your WSIB Provider ID, invoice number, service date, name, address and telephone. The service code is pre-printed as 8M; the chiropractor, physiotherapist and nurse practitioner fee schedules list 8M1 for this report. Complete the HST fields (code ONHST) only if HST applies. WSIB lists an incomplete, illegible or incorrect Section E as a cause of payment delays.

F. Return To Work Information: must be completed by a Health Professional

This is page 2, the only page the employer sees. Record whether you discussed return to work, then tick regular or modified duties with start dates and any graduated hours, or explain why the worker cannot work. Mark functional abilities (bend/twist, climb, kneel, lift, sit, stand, walk, push/pull, upper extremities, driving, heavy equipment, public transit), other limitations, how long they will apply (from 1 to 2 days up to 14+ days) and the follow-up plan, then print your name and sign.

G. Worker's Signature

The worker signs and dates this box to authorize you to give their employer a copy of page 2 outlining their functional abilities, and acknowledges that a copy goes to the WSIB. The page repeats the worker's name, birth date, areas of injury and incident date at the top so it can be handed over on its own. Give the worker a copy of this page only, never page 1.

Common mistakes

  • Sending the report with Section A incomplete, illegible or incorrect, which WSIB names as a main reason Form 8 payments are delayed.
  • Leaving Section E incomplete or wrong, such as a missing WSIB Provider ID, which also holds up payment of the form fee.
  • Sending only one page. The WSIB needs both report pages, and payment is delayed when only one page is received.
  • Filing a second Form 8 for the same person or using it as a progress report; the WSIB requests progress on Form 26.
  • Completing a Functional Abilities Form at the initial visit: only the Form 8 is paid that day, and an FAF dated the same day is not.
  • Giving the employer page 1, which holds clinical findings and the diagnosis; only page 2 goes to the worker to pass on.

Submitting and payment

How to submit it
Send both report pages to the WSIB electronically through TELUS Health or by fax or mail, and give the patient a copy of page 2 only for their employer. WSIB calls prompt reporting critical to adjudicating the claim, and may reduce accounts it receives more than six months after the service date.
Who pays
The WSIB pays the form fee directly: physicians bill 8M (paper) or 8ME (electronic), and chiropractors, physiotherapists and nurse practitioners bill 8M1 or 8M1E, with electronic submission paying a higher fee and paying faster.

Good to know

  • Submit only one Form 8 per person and never use it as a progress report; the WSIB requests updates on Form 26, and a narrative report on letterhead bills as code 26.
  • No patient consent is needed: section 37 of the Workplace Safety and Insurance Act authorizes health professionals to send this report whether the patient says the injury is work-related or you suspect it.
  • Work-related mental stress is reported on Form CMS8 by physicians or nurse practitioners; if the patient also has a physical injury or illness, a Form 8 can be completed the same day.
  • On the initial visit only the Form 8 is paid. A Functional Abilities Form completed on the same date is not paid, and the FAF is only done when the worker or employer asks.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When do I have to complete a WSIB Form 8?

After the initial visit for a work-related physical injury or illness, even if the patient was first seen in an emergency department, and whenever you believe workplace factors caused the condition. WSIB says prompt submission is critical to processing the claim. Physicians can also complete a Form 8 when a patient reports a recurrence of a previous work-related injury or illness.

Do I need my patient's consent to send Form 8 to the WSIB?

No. Section 37 of the Workplace Safety and Insurance Act authorizes health professionals, hospitals and health facilities to submit claim information to the WSIB without consent, and PHIPA also permits the disclosure. The patient's signature in Section G covers a different step: it authorizes you to give their employer a copy of page 2, the return-to-work information.

Will the WSIB pay for a Form 8 if the patient has no claim?

Yes. When a worker cannot be assigned a claim, for example because the employer is not registered or not covered under the Act, the WSIB makes a one-time payment for the Form 8 only, not for ongoing care. Complete all patient identifiers, include your nine-digit provider number, name and address, and sign the form.

What billing code do I use for WSIB Form 8?

Physicians bill 8M for a paper Form 8 and 8ME when it is submitted electronically through TELUS Health. Chiropractors, physiotherapists and nurse practitioners use 8M1 (paper) or 8M1E (electronic), even though the billing section is pre-printed with 8M. WSIB says electronic submission brings a higher fee and faster payment.
