WSIB Form 26 (0896A): Progress Report Guide

When a case manager needs an update on recovery and return to work, the WSIB sends this one-page report to the worker, who brings it to a treating health professional. It asks about work status, functional abilities, change since the last visit, current diagnosis, prognosis and treatment. The WSIB pays for it when the claim is allowed, with separate fee codes for paper and electronic submission.

Who completes it

The treating physician, nurse practitioner (RN Extended Class), chiropractor or physiotherapist completes the whole form, answering every question in black ink or type. The worker brings the WSIB-issued form but has no section to complete.

How to fill it in

Claim Number, Worker's name and Date of Incident

Confirm the claim number, worker's name and date of incident (dd/mmm/yyyy) at the top of the WSIB-issued form. The form notes that section 37 of the Workplace Safety and Insurance Act authorizes you to release this information to the WSIB. It asks you to answer all questions in black ink or type, so every item below needs a response.

Return to Work Information: question 1

Tick whether the worker can resume regular duties or begin modified duties, with a start date and any graduated hours. The form reminds you that returning to safe and appropriate work is best practice and that most workers with soft tissue injury can remain at work. If the worker cannot return, it states that pain should not be the only medical restriction: give any other reason, with details and an expected return-to-work date.

Return to Work Information: question 2

Indicate the worker's functional abilities in relation to the workplace injury: full functional abilities, or some abilities, marking able or not able to bend/twist, climb, kneel, lift, operate heavy equipment, operate a motor vehicle, push/pull, sit, stand, use public transportation, use upper extremities and walk. Tick other limitations due to environmental conditions, medication or protective equipment, and add maximum repetitions, weights or times in the comments line.

Clinical Information and Treatment Plan: questions 3 to 5

Record the change in the patient's condition since the last visit (recovered, improving, worsening or unchanged), with details if worsening. Give the current diagnosis. Then state whether you are aware of pre-existing or other conditions or factors that would affect return to work or recovery, and describe them if so; the form gives psychosocial factors and medications as examples.

Clinical Information and Treatment Plan: questions 6 and 7

Select one prognosis: fully recovered now; partially recovered and continuing to improve; full recovery not yet known; partially recovered with full recovery expected in a stated number of weeks; or full recovery not expected. Then describe the current treatment plan, including the type of treatment, interventions and duration. Together with the return-to-work answers, this is the follow-up information the case manager asked for.

Billing Section

Tick your designation (chiropractor, physician, physiotherapist or registered nurse in the extended class), then enter your WSIB Provider ID, invoice number, service date, name, address, telephone and fax, and sign. The service code is pre-printed as 26M; the chiropractor, physiotherapist and nurse practitioner fee schedules list 26M1 for this report. Use the ONHST code and amount only if HST applies.

Common mistakes

  • Printing the PDF from wsib.ca: it is marked for reference only and cannot be printed. Use the WSIB-issued form or TELUS Health.
  • Altering or reusing a previously completed Form 26 as a new report, which the WSIB physician fee schedule prohibits.
  • Listing pain as the only restriction when the worker cannot return; the form asks for any other reason, with details and an expected return date.
  • Sending another Form 8 as an update. Form 8 is the initial report only and must never be used as a progress report.
  • Leaving questions unanswered or writing in a colour other than black; the form asks for every question to be answered in black ink or type.
  • Billing late: accounts the WSIB receives more than six months after the service date may be reduced, up to no payment after 12 months.

Submitting and payment

How to submit it
Fax the completed WSIB-issued form to the WSIB, or submit the electronic version through TELUS Health, which pays a higher fee. Submit promptly, since the WSIB may reduce accounts it receives more than six months after the service date.
Who pays
The WSIB pays physicians under 26M (paper) or 26ME (electronic) and chiropractors, physiotherapists and nurse practitioners under 26M1 or 26M1E once the claim is allowed, while an unsolicited narrative progress report on letterhead is code 26.

Good to know

  • The Form 26 PDF on wsib.ca is marked for reference only and cannot be printed; complete the paper form the WSIB issued or use the TELUS Health electronic version.
  • A previously completed Form 26 must not be altered or reused as a new report, according to the WSIB physician fee schedule.
  • Between requests, you can send a narrative progress report on letterhead (code 26) when you become aware of new or significant clinical information about the work injury.
  • The WSIB may cut payment for accounts received more than six months after the service date, rising to no payment after 12 months, unless you give a good reason for the delay.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When do I fill out a WSIB Form 26?

When the worker brings you one. The WSIB sends Form 26 to the person with the work-related injury or illness when its case manager needs follow-up information, and you complete and submit it. It is not used for the first report: that is Form 8, which must never be used as a progress report.

Can I print Form 26 from the WSIB website?

No. The PDF posted on wsib.ca is stamped as a reference-only version that cannot be printed. Complete the paper form the WSIB issued to the worker, or use the electronic version through TELUS Health. Do not alter or reuse a Form 26 you have already completed as a new report.

Can I send a progress report without a Form 26?

Yes. When you become aware of new or significant clinical information, you can send a narrative progress report on your letterhead, billed under code 26. It is separate from the Form 26 the WSIB requests. Physicians can also ask to speak with a WSIB staff physician about a patient's claim.

What are the billing codes for WSIB Form 26?

Physicians bill 26M for a paper Form 26 and 26ME for an electronic one. Chiropractors, physiotherapists and nurse practitioners bill 26M1 or 26M1E. The billing section is pre-printed with 26M, and the WSIB pays the progress report when the patient has an allowed claim. Electronic submission pays a higher fee.
