WSIB forms for Ontario physicians, NPs and therapists

Form 8, Form 26, the Functional Abilities Form and the WSIB's program of care, hearing and medication forms, with who completes each one and how it reaches the WSIB.

WSIB is Ontario's workers' compensation board. The form most clinicians meet first is the Health Professional's Report (Form 8), completed after the initial visit for a work-related injury or illness. Progress goes on the Health Professional's Progress Report (Form 26), and the Functional Abilities Form (2647A) helps plan an early and safe return to work. Physiotherapists and chiropractors treating musculoskeletal injuries in the community report through the MSK program of care forms (10635A to 10638A).

A physician, nurse practitioner (NP), chiropractor or physiotherapist can complete Form 8, once per person and never as a progress report. For occupational mental stress, physicians and NPs use Form CMS8 instead. The WSIB pays a higher fee when Form 8 or Form 26 goes electronically through TELUS Health, and most other reports go through WSIB online services. Some forms, such as Form 8D, come from the WSIB for a specific patient. Vero Documents fills WSIB PDFs from the visit note and the chart.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who completes WSIB Form 8?

A physician, nurse practitioner, chiropractor or physiotherapist completes Form 8 (0008A) after the first visit for a work-related physical injury or illness, or when workplace factors are suspected. The patient fills in Section A, and page 2 goes to the employer through the patient. Submit only one Form 8 per person. Section 37 of the WSIA authorizes the release without the patient's consent.

When should I complete the WSIB Functional Abilities Form?

Only when the employer or the worker asks for it, because health professionals cannot initiate the FAF (2647A). It describes functional abilities and restrictions with no diagnosis or medical information. Any regulated health professional can complete it, from physicians and NPs to physiotherapists and psychologists. The WSIB pays for it, except when it is done on the same date as the initial Form 8. If the employer uses its own form, the employer pays.

Where do I get a WSIB Form 26?

From the patient, who brings the WSIB-issued form to the visit. The Form 26 (0896A) PDF on the WSIB site is marked for reference only and cannot be printed. Complete the paper form without altering or reusing a completed one, or use the electronic version through TELUS Health, which pays a higher fee. Physicians, NPs, chiropractors, physiotherapists and RNs (Extended Class) can complete it.

Which WSIB forms are sent per case?

Several reports for physicians and NPs are not posted publicly. The WSIB sends them when needed: Form 8D (occupational disease), 8D2 (skin disease), 8S (chest disease), 8W (vibration-induced white finger), the home oxygen therapy report, the cannabis initial and follow-up assessments (0210A, 0211A), the FAE medical clearance (0298A) and the opioid assessment form. Complete the copy you receive and return it to the WSIB.
