Provincial disability assistance forms for clinicians

Medical reports for provincial and territorial income and disability assistance, from ODSP and AISH to BC's PWD designation and Quebec's social assistance programs.

Provincial and territorial assistance programs ask clinicians to confirm a disability, a limit on the ability to work, or a medical need for extra benefits. The main disability reports are Ontario's ODSP Disability Determination Package (the Health Status Report and the Activities of Daily Living Index), Alberta's Disability Income Assistance Medical Report (DS2444B) for AISH and ADAP, and BC's Persons with Disabilities Designation Application (HR2883). Quebec's social assistance uses the Rapport d'évaluation médicale ou psychosociale (SR-2100), and the Northwest Territories uses Form D (NWT9386).

Who pays depends on the province. Ontario physicians bill OHIP fee codes for the ODSP reports and BC physicians bill MSP fee items for the PWD application, while other professionals invoice the ministry. In Alberta the applicant pays for the DS2444B, though Income Support, AISH or ADAP may cover it. Several forms come only from the program: the paper ODSP package, the working copies of several BC ministry forms and New Brunswick's medical reports reach you through the patient or caseworker.

Frequently asked questions

How do I complete the ODSP Health Status Report?

Online through SADIE, which needs a GO Secure account, an OHIP billing number and the patient's Member ID and Referral ID, or on the paper package ODSP issues to the applicant. A physician, NP, registered nurse, psychologist or psychological associate, or optometrist can complete it. The Activities of Daily Living Index can be done by the same or a different professional, and the patient has 90 days to submit.

Who can complete BC's PWD designation application?

A medical practitioner or nurse practitioner completes Section 2, the Medical Report. Section 3, the Assessor Report, can be done by a medical practitioner, NP or another prescribed professional: a registered nurse or psychiatric nurse, occupational therapist, physical therapist, psychologist, chiropractor or registered social worker. Different professionals may complete the two reports. Physicians bill MSP fee codes 96501 and 96502; other professionals invoice the ministry.

What medical report does AISH need?

AISH and the Alberta Disability Assistance Program (ADAP) share one Disability Income Assistance Medical Report (DS2444B), which replaced the old AISH Application Part B. A health professional registered in Alberta, normally the applicant's physician, reports on diagnosis, functioning, limitations and prognosis. The fillable PDF must be opened in Adobe Reader. It goes to the applicant to upload with the online application, or it can be sent by email, fax or mail.

Which Quebec social assistance forms do clinicians complete?

The Rapport d'évaluation médicale ou psychosociale (SR-2100) documents a person's diagnosis and functional limitations so the ministry can recognize health-related employment constraints. A physician, specialized nurse practitioner, psychologist, social worker or couple and family therapist can complete it, and with the person's consent you may send it to the ministry yourself. The Certificat médical (SR-0040) covers pregnancy and special medical benefits. Both forms are in French.
