Manitoba Wheelchair Seating Component Request Form

Manitoba's Disability and Health Supports Unit (DHSU) uses this form to fund wheelchair seating components, such as cushions, backrests and secondary positioning parts, for people in Employment and Income Assistance, Manitoba Supports for Persons with Disabilities, Children's disABILITY Services or Community Living disABILITY Services. The therapist assesses the client, trials products from an authorized vendor and submits the final prescription with the vendor's quote.

Who completes it

An occupational therapist or physiotherapist licensed to practise in Manitoba completes the whole form as the prescriber. The selected authorized vendor supplies the trial equipment and an itemized quote for the final prescription.

Submitting and payment

How to submit it
Send the completed form with the authorized vendor's quote to DHSU Provincial Services by fax, email or mail, as printed on page 1. Incomplete forms are returned.
Who pays
Neither the form nor the DHSU seating manual sets a fee for the prescribing therapist; the Materials Distribution Agency (MDA) pays the vendor after the prescriber signs off the invoice.

Good to know

  • Seating is funded only when a permanent mobility base is confirmed, DHSU funds one seating system per client, and the prescription must be the most economical option that meets basic and essential needs.
  • For extra components on a request submitted within the last 6 months, complete only Sections 1, 2, 8 and 9 and give the original request date.
  • Components are not replaced for preference, loss or theft, or while still adequate; replacement needs a size change, a significant status change or wear that puts the client at risk.
  • Repairs or replacement of the same exact part go directly to MDA, not through this form. Vendor fittings and adjustments are free within 4 weeks, or 12 weeks for custom-fabricated components.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete the Manitoba Wheelchair Seating Component Request?

Only an occupational therapist or physiotherapist licensed to practise in Manitoba. DHSU's seating manual makes the prescriber responsible for the assessment, choosing a vendor from the Wheelchair Seating Component Equipment List, arranging the trial, submitting the form with the quote, following up with the client and signing off the vendor's invoice.

What happens after DHSU approves a seating request?

DHSU notifies the client and the prescriber of the outcome. If the request is approved, the equipment is ordered through MDA, which issues a purchase order to the selected vendor. The vendor supplies only what is approved and invoices MDA after the prescriber signs off the completed work.

Is a product trial required for wheelchair seating?

DHSU's form guide calls the product trial crucial for showing that the targeted outcomes are achieved. Section 8 records each product trialled, whether the trial succeeded and the outcome, and the prescriber must give a reason if nothing was trialled. Custom modifications need a clinical rationale in Section 9.
