Frequently asked questions

What does the Canadian medical forms library cover?

Third-party forms that clinicians complete in Canada: workers' compensation reports for all 12 boards, federal forms such as the T2201, CPP disability and EI certificates, and auto insurance forms from ICBC, the SAAQ, MPI, SGI, Alberta and Ontario's OCF series. It also covers disability assistance, student aid, drug coverage, driver medicals, accessible parking, Veterans Affairs Canada and private insurers, grouped into federal, provincial and territorial hubs.

How were the forms in the library verified?

Each entry was checked against the issuer's official page or PDF in October 2026, and only verified entries are listed. Each row shows the form name and number, who completes it, its format and a link to the official page or PDF. Issuers revise forms from time to time, so open the official link and confirm you have the current version before you submit.

Are French forms included?

Yes. French versions exist for 342 entries. Quebec's CNESST, IVAC and RAMQ forms are French only, as are some SAAQ clinician forms, while most federal, Ontario and New Brunswick forms come in both languages. For example, the CNESST's first medical certificate, the Attestation médicale (1936), exists only in French.
