CPP disability medical report and related CPP and QPP forms

Service Canada's Canada Pension Plan disability forms for physicians and nurse practitioners, plus Retraite Québec's medical report for QPP disability benefits.

For a Canada Pension Plan disability (CPP-D) application, the treating physician or nurse practitioner completes Sections 3 to 9 of the Medical Report for Canada Pension Plan Disability Benefits (ISP-2519), on conditions, functional limitations, treatment, employment and prognosis; the patient completes Sections 1 and 2. A terminal illness uses the short attestation ISP-2530B instead. Service Canada sends some reports directly to clinicians, such as the Reassessment Medical Report (ISP-2509). Québec Pension Plan (QPP) disability benefits use Retraite Québec's Medical Report (B-076).

Send the signed ISP-2519 to Service Canada, or return it to the patient to upload in My Service Canada Account, mail or drop off. Two forms deal with capacity: the ISP-1800 declares that a patient could not form or express an intention to apply, so the application can be treated as made earlier, and the ISP-3505 supports appointing a trustee for the patient's OAS or CPP benefits. Vero Documents fills the ISP-2519 from the visit note and the chart; you review and sign.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who pays for the CPP disability medical report?

Service Canada pays the physician or nurse practitioner directly for the ISP-2519, up to a set amount, and the patient pays anything above it. Mail the invoice to the Service Canada office listed on the form, with the patient's name, address and ID number and your business, GST/HST or SIN number. Service Canada also pays for the ISP-2530B, the ISP-1800 incapacity declaration, and the ISP-2509 and ISP-2525 reports it asks you to complete.

Can I send clinical notes instead of filling in the whole ISP-2519?

Yes, for patients you follow closely. You may submit detailed clinical notes and complete only Section 5 (conditions, impairments, limitations and treatment) and Section 9 (the declaration). If the patient already has a disability claim with an insurer or a provincial agency, that body can mail its recent medical reports to Service Canada instead.

Which CPP disability form is used for a terminal illness?

The Terminal Illness Medical Attestation for a Disability Benefits Application Under the Canada Pension Plan (ISP-2530B), which replaces the ISP-2519 for these applicants. The patient completes Sections 1 and 2, and a physician or NP completes Section 3, a short attestation for fast-tracked benefits. On the standard ISP-2519, Section 4 requests expedited processing for terminal or grave conditions.

How does the Québec Pension Plan medical report work?

Retraite Québec's Medical Report for the Québec Pension Plan (Rapport médical, form B-076; English version B-076A) is completed by the physician after the applicant fills sections 1 and 2. One form covers new applications and medical re-assessments. It can be sent online or by mail, and an electronic signature is accepted. The exam is insured by RAMQ, and any fee for writing the report is billed to the applicant.
