ISP-1800: CPP Declaration of Incapacity (Physician)

Service Canada uses this report when a person claims they were unable to apply for Canada Pension Plan benefits on time. The physician declares that the patient could not form or express an intention to apply and gives the start and end dates of that incapacity. If accepted, the application can be treated as made earlier than the normal retroactivity limit would allow.

Who completes it

The physician completes the whole report: the dates of incapacity, the medical condition and an opinion on the patient's capacity. Social Security Tribunal decisions show that nurse practitioners have also completed it.

Submitting and payment

How to submit it
Return the report to the Service Canada office listed on the form and mail your invoice to that same office.
Who pays
Service Canada pays the clinician by cheque, up to the maximum set in its reimbursement table for health professionals, and the patient covers any extra cost.

Good to know

  • Service Canada's forms catalogue lists ISP-1800 without a public download, so clinicians receive the form from Service Canada rather than printing it themselves.
  • Your invoice must show the patient's name, address, birth date and SIN, plus your or your clinic's full name and a business number, GST/HST number or SIN.
  • Its companion is a CPP benefit application, for example the ISP-1151 Application for Canada Pension Plan Disability Benefit.
  • Service Canada asks clinicians to answer every question on any form it sends and to call if they cannot provide the medical information requested.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Where can I download the ISP-1800 Declaration of Incapacity?

You cannot. The Service Canada forms catalogue lists the title in English and French but offers no public download, so Service Canada supplies the form on request. Contact the Service Canada office handling the application if you need a copy to complete.

Can a nurse practitioner complete the ISP-1800?

The report is titled a physician's report, and Service Canada's fee table refers to it as the Declaration of Incapacity, Physician's Report. Social Security Tribunal decisions show nurse practitioners have also completed it. If you are unsure whether your signature will be accepted, ask the Service Canada office that sent it.

Is the Declaration of Incapacity available in French?

Yes. The French title is Déclaration d'incapacité - rapport du médecin, and Service Canada lists the form in both official languages. Because it is supplied directly rather than downloaded, ask the Service Canada office handling the file for the French version if your patient needs it.
