ISP-2525: CPP Recurrence of Same Medical Problem

Service Canada requests this short report when a patient's disabling condition comes back, in connection with Canada Pension Plan disability benefits. The treating clinician describes the recurrence so Service Canada has current medical evidence for its decision. The form is not public: it arrives from Service Canada about a specific patient, and you return it to the office listed on it with an invoice.

Who completes it

The treating physician or nurse practitioner who receives the request from Service Canada answers the report's clinical questions.

Submitting and payment

How to submit it
Return the report to the Service Canada office listed on the form and mail your invoice there as well.
Who pays
Service Canada pays the clinician by cheque, up to the maximum its reimbursement table sets for the recurrence report, and patients cover any amount above that.

Good to know

  • Automatic reinstatement applies when benefits stopped less than 2 years ago, the same or a related disability recurs, and the person is under 65 and not receiving a CPP retirement pension.
  • Automatic reinstatement needs a form from the doctor or nurse practitioner confirming the disability has recurred; Service Canada mails the forms to the beneficiary when benefits stop.
  • People who worked longer than 2 years can ask for a fast-track reapplication, available up to 5 years after benefits stopped, under the regular eligibility conditions.
  • Your invoice must list the patient's name, address, birth date and SIN, with your full name or your clinic's and a business number, GST/HST number or SIN.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

When does a patient need a recurrence report for CPP disability?

When CPP disability benefits stopped because the person returned to work, volunteering or school, and the same or a related disability comes back, Service Canada can restart benefits through automatic reinstatement. That process requires the beneficiary's request and a form from the doctor or nurse practitioner confirming the recurrence.

How much information does the ISP-2525 need?

It is a short report, paid at a much lower maximum than the full ISP-2519 medical report. Service Canada asks that every question be answered, and its general guidance asks reports to cover conditions, functional limitations, prognosis, treatment and the impact on the patient's ability to work now and in the future.

Can I download the ISP-2525?

No. The ISP-2525 is not in Service Canada's public forms catalogue. Service Canada sends its recurrence and reinstatement forms itself; for automatic reinstatement, it mails the forms to the beneficiary when benefits stop. A patient who needs a copy should contact Service Canada directly.
