ISP-3505: Certificate of Incapability (OAS/CPP)

When someone receiving Old Age Security or Canada Pension Plan benefits can no longer manage their own affairs, a trustee, agency or institution can apply to receive and administer those benefits on their behalf. Service Canada uses this certificate to decide whether the person is incapable. To qualify, the person must have a severe mental impairment or a physical illness or impairment.

Who completes it

A medical professional (physician, registered nurse, nurse practitioner, psychologist or psychiatrist) answers questions 1 to 9; for seniors, a designated social worker, lawyer or member of the clergy may certify using questions 10 and 11 instead. The person or agency applying to administer the benefits completes a separate agreement, ISP-3506 or ISP-3507.

Submitting and payment

How to submit it
The completed certificate is mailed to the nearest Service Canada office listed on the form's address page, or, from outside Canada, to the office for the province where the person last lived.
Who pays
The ISP-3505 does not appear in Service Canada's reimbursement table for health professionals, which lists the CPP disability reports it pays for.

Good to know

  • Questions 1 to 5 ask, with yes/no and comments, about knowledge of money, sense of time for paying bills, memory for transactions, ability to balance accounts, and impaired judgment.
  • Anyone related by blood or marriage to the person, or to the proposed administrator, cannot certify incapability.
  • Question 12, a membership and registration number certification, applies when certifying a senior who is homeless or at imminent risk of homelessness.
  • It pairs with ISP-3506 (agreement for a private trustee) or ISP-3507 (agreement for an agency or institution) to administer the benefits.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

What does the medical professional write on the ISP-3505?
