Federal medical forms for clinicians in Canada

Forms from the CRA, Service Canada, Veterans Affairs Canada, ESDC, Health Canada, NIHB, Transport Canada, IRCC and the federal public service disability plans.

Most forms in this list come from federal departments and agencies, and the rest from the insurers for federal employees' disability plans. For primary care, the key ones are the CRA's Disability Tax Credit Certificate (T2201), Service Canada's Medical Report for CPP disability benefits (ISP-2519), the EI sickness certificate (INS5140) and Veterans Affairs Canada's medical questionnaires. Others cover Health Canada's Special Access Programs and cannabis document, NIHB drug and counselling requests, aviation and marine medicals, immigration and citizenship, and student loans.

Payment differs by program: Service Canada pays the doctor or NP for CPP disability reports and VAC pays for its questionnaires, while the CRA does not pay for the T2201 and the applicant pays any fee. Some forms are limited to designated examiners, such as IRCC panel physicians. For long-term disability claims, the Public Service Disability Insurance Plan and the PSMIP use the Initial Disability Insurance Medical Statement, and SISIP uses attending physician's statements for Canadian Armed Forces members.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can any doctor complete an IRCC immigration medical exam?

No. Only IRCC-designated panel physicians complete immigration medical examinations, recorded in eMedical on the Medical Report: Client Biodata and Summary (IMM 1017). The paper Medical Report (IMM 5419) is used only where eMedical is not available, with Regional Medical Office approval. A patient's own GP, specialist, psychologist or nurse practitioner can, however, complete the Medical Opinion Form for Citizenship Waivers (CIT 0547), which supports a request to waive the language, knowledge or oath requirements.

Can a family doctor sign a Transport Canada aviation medical?

Partly. The Civil Aviation Medical Examination Report (26-0010) is restricted to Civil Aviation Medical Examiners. But any family physician can countersign Part B of the Category 4 Medical Declaration (26-0297) for a student pilot permit for aeroplanes or a recreational pilot permit, unless the applicant has had a condition listed in Part B. Any physician or optometrist can report an aviation-relevant condition on form 26-0011.

How do I request a drug through Health Canada's Special Access Program?

Use Form A for a specific patient with a serious or life-threatening condition who needs a drug not marketed in Canada. Use Form B to request a drug ahead of time for future emergency use when the patient is not yet known. After using the drug, file Form C, the mandatory follow-up report on results and adverse reactions. The practitioner completes each form and faxes it without a cover sheet.

Is there a required form for authorizing medical cannabis?

No. Health Canada posts a sample Medical document authorizing the use of cannabis for medical purposes, but any document with the information required by section 273 of the Cannabis Regulations is acceptable. The original goes directly to the licensed seller the patient chooses or, for personal or designated production, to Health Canada with the registration application.
