OSAP disability verification form and other student aid forms

Disability verification forms for post-secondary student aid in Ontario and other provinces and territories, plus the federal forms for student loan relief.

Students with a permanent, or persistent or prolonged, disability need a clinician's verification to qualify for disability grants and related help from student aid. In Ontario that is the OSAP Disability Verification Form, reissued every academic year in two versions: one for Ontario public postsecondary institutions, and one for private schools or OSAP-approved schools outside Ontario. Students elsewhere apply through their own province or territory, with its own form. Federally, ESDC's Severe Permanent Disability Benefit Medical Report (SDE0061 MED) supports cancelling student loans.

On the OSAP form you complete Section B, with your office stamp or signed letterhead. Any blank field in Section B means the student will not be considered disabled, and the form is normally completed only once per student. The student uploads it to their OSAP account or sends it to the school's financial aid office. Elsewhere, New Brunswick will not accept a Section B completed electronically and needs a wet signature, and Quebec's Aide financière aux études uses a French Certificat médical (form 1015).

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete the OSAP Disability Verification Form?

A physician, nurse practitioner or other regulated health care professional whose scope includes diagnosing. The form's checklist covers audiologists and speech-language pathologists, chiropractors, neurologists, occupational therapists, optometrists, ophthalmologists, family physicians, psychiatrists, other physicians, physiotherapists, psychologists, psychological associates, neuropsychologists and rheumatologists. The student completes Section A and you complete Section B.

When is the OSAP disability verification form due?

OSAP must receive it no later than 40 days before the end of the student's study period. The form is reissued for each academic year, so use the current version. Students at private Ontario schools or OSAP-approved schools outside Ontario need the private-institution version, which adds a part for recommended accommodations and supports; if that part is left blank, the student may not qualify for the related grants.

Is a psycho-educational assessment needed for a learning disability?

In several provinces, yes. BC, New Brunswick and Nova Scotia use a psycho-educational assessment by a registered psychologist instead of the clinician's verification, while Alberta, Manitoba and Newfoundland and Labrador ask for one along with the form. BC, Manitoba and New Brunswick want the assessment done within the last 5 years or at age 18 or older.

Which federal student loan forms need a clinician?

This list has two ESDC forms. The Severe Permanent Disability Benefit Medical Report (SDE0061 MED), completed by a physician or NP, supports cancelling Canada Student Loans, participating provincial loans and Canada Apprentice Loans; it must be signed within the last 12 months, and the patient pays any fee. The verification for the Repayment Assistance Plan for Borrowers with Disabilities (SDE0083) is for first-time applicants who have never received a disability grant.
