AFE 1015: Certificat médical pour personne aux études

Aide financière aux études (AFE) uses this three-page French certificate to confirm that a student has a physical or mental impairment causing a significant and persistent disability. It supports accommodations in the Loans and Bursaries Program, such as aid entirely as a bursary, aid between study periods and part-time eligibility, and applications to the Allowance for Special Needs Program. A new edition is issued for each aid year.

Who completes it

A physician completes Section 2, the diagnosis, and Section 3, the assessment of disability and barriers, can be completed by a physician or another professional under the Professional Code with the required skills, such as a psychologist for mental functions or an optometrist for vision. The student completes Sections 1 and 4.

Submitting and payment

How to submit it
The student uploads the completed certificate to their Aide financière aux études file, following the upload procedure on AFE's website.

Good to know

  • For the Loans and Bursaries Program, Sections 1 to 4 must be completed; for the Allowance for Special Needs Program, Sections 1, 2 and 4 are enough.
  • A significant disability causes major barriers despite aids such as a prosthesis, medication or therapy; a persistent one does not change over time or come from an injury or illness that will resolve.
  • Section 3A covers labour market integration in summer, between study periods and after studies; Section 3B covers full-time study for the 2026-2027 year and for the whole program.
  • Each part of Section 3 requires the date the incapacities began, and the form states that electronic signatures are not accepted.

How this list is checked

The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete the AFE medical certificate (form 1015)?

Section 3 may be completed by a physician or by a professional under Québec's Professional Code with the skills for the assessment: for mental functions, a physician, psychologist or specialized nurse practitioner in mental health; for vision, an optometrist.

What can the AFE medical certificate unlock for a student?

In the Loans and Bursaries Program, it can allow aid entirely as a bursary, aid between two study periods such as the summer, and continued eligibility while studying part-time.
