StudentAid BC Disability Verification Form

StudentAid BC uses this four-page form to verify a permanent disability (PD) or a persistent or prolonged disability (PPD) for six programs, including the Canada Student Grant for Students with Disabilities and the B.C. Access Grant for Students with Disabilities. The assessor describes the primary and any secondary disability, the functional limitations and how they restrict post-secondary study. Giving a specific diagnosis is optional.

Who completes it

A qualified medical assessor registered in the Canadian province or territory where the assessment is done completes it: a physician, nurse practitioner or psychologist for most conditions, an ophthalmologist, optometrist or orthoptist for visual conditions, or a certified audiologist for auditory conditions.

Submitting and payment

How to submit it
The student uploads the complete, signed form to their StudentAid BC account under Forms, Student Forms, Standard Forms, Disability status application; documents sent through the main File Uploader tab are not accepted or processed.

Good to know

  • Visual conditions also need a copy of the most recent visual acuity report, and auditory conditions a copy of the most recent audiology report, along with the form.
  • Specific learning disorders need a psycho-educational assessment by a registered psychologist, done in the last five years or at 18 or older, instead of this form.
  • Students with verified PD status do not re-verify; students with PPD re-verify when they enter loan repayment or after a break in study of more than 5 years from the original assessment.
  • Write the student's name on each page, answer every question, and check limitations such as attending classes, focus and concentration, taking notes, reading and remembering information.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can sign the StudentAid BC Disability Verification Form?

A qualified medical assessor in Canada who is registered to practise in the province or territory where the assessment is done. Physicians, nurse practitioners and psychologists cover most disabling conditions; ophthalmologists, optometrists or orthoptists sign for visual conditions, and certified audiologists for auditory conditions.

Does the assessor have to give a diagnosis?

No. The assessor instructions say providing the specific diagnosis is optional. The assessor must describe the disability and its functional limitations, confirm they are permanent or persistent or prolonged, and explain how they restrict the daily activities needed to study at a post-secondary level or join the labour force.

Does a student need a new form every year?

Not if StudentAid BC has already verified a permanent disability. Students with a verified persistent or prolonged disability must re-verify when they enter loan repayment or after a break in study of more than 5 years from the original assessment, and StudentAid BC may request updated documentation.
