Manitoba Student Aid Verification of Disability Form

Manitoba Student Aid uses this form to verify a permanent disability or a persistent or prolonged disability, which opens Canada Student Grants for students with disabilities, extra weeks of funding and a reduced course load. A qualified medical assessor identifies each disability type, its functional limitations and how they make post-secondary education difficult, and may recommend accommodations. It is due 45 days before the study period ends.

Who completes it

A qualified, registered medical assessor licensed where the assessment is done, such as a physician, nurse practitioner, audiologist or psychologist, completes Sections B and C. The student completes Section A with their file number, school, program and study period dates.

Submitting and payment

How to submit it
The completed form goes to Manitoba Student Aid by mail or email no later than 45 days before the end of the student's study period.
Who pays
Students pay for medical documentation upfront and may be reimbursed, up to a yearly maximum, if they paid no earlier than four months before the study period and apply for the Canada Student Grant for Services and Equipment.

Good to know

  • If the applicant does not have a disability meeting the definitions, the assessor skips the rest of Section B and moves on to Section C, identification and signature.
  • Electronic signatures, official watermarks or stamps are acceptable, and extra information can be attached as a letter if more space is needed.
  • Instead of this form, Manitoba Student Aid accepts a school medical form, medical documentation or a signed letter from a qualified assessor, or proof of permanent disability assistance.
  • Students with a permanent disability already verified by Manitoba Student Aid do not resubmit, unless claiming an additional disability or returning after a gap with a persistent or prolonged one.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

What is the deadline for Manitoba Student Aid's Verification of Disability Form?

45 days before the end of the student's study period. Funds cannot be disbursed after the last day of the study period, and processing may take six to eight weeks or longer during peak periods, so the form should be completed and returned well before the deadline.

Can a letter be used instead of the Manitoba form?

Yes. Manitoba Student Aid accepts a signed letter or report from a qualified medical assessor if it gives the same information: the type of disability and its functional limitations, the expected duration (permanent or at least 12 months), and how the disability makes post-secondary study or labour force participation difficult.

Is the Manitoba Verification of Disability Form available in French?

Yes. Manitoba Student Aid publishes a French version, Formulaire Vérification de l'invalidité, alongside the English form revised August 2026. The English form is not fillable, so the assessor prints clearly, answers all questions in full and attaches a letter if more space is needed.
