OSAP Disability Verification Form 2026-27 (Private)

Students at private schools in Ontario, or at OSAP-approved schools outside the province, use this edition to have a permanent or a persistent or prolonged disability verified for OSAP and the Ontario Learn and Stay Grant. Your Section B matches the public-school edition except for Part 6, where you recommend accommodations or supports. The BSWD and CSG-DSE grants can help fund those recommendations, although funding isn't guaranteed.

Who completes it

A physician, nurse practitioner or other regulated health care professional whose scope of practice includes diagnosing completes Section B, Parts 1 to 7. The student completes Section A, including the required consents and an optional consent to share the disability information with the school's accessibility office, if there is one.

How to fill it in

Section A: Student information, consents and declarations

The student supplies their school, Social Insurance Number, student number, Ontario Education Number if they have one, and contact details, and signs the required consents in Part 1. Those consents let you send the health information to the ministry and the school, and let either contact you about unclear or illegible answers, but only about this form. An optional Part 2 consent lets the school's accessibility office see the disability details, where the school has such an office.

Section B: Verification of patient's disability

Begin with the patient's first name, last name and date of birth. The ministry weighs whether the disability meets the federal definitions and causes functional limitations that restrict the daily activities needed for postsecondary study. Give clear statements on the nature of the disability, the limitations and the educational barriers, and continue on official letterhead if you run out of space. Parts 1 to 5 and 7 must be fully answered; Part 6 affects grant eligibility rather than verification.

Part 1: Physician or regulated health care professional information

Provide your name, telephone number and specialty, and tick each provider type that applies, such as nurse practitioner, optometrist, physiotherapist, neurologist or psychologist. The licence chart needs your Canadian provincial or territorial licence number and address, plus an office stamp; with no stamp, sign your letterhead and attach it. The form will not be accepted if the chart is incomplete and has neither a stamp nor signed letterhead.

Part 2: Patient's disability status

Part 2 holds two Yes/No questions. Permanent means the impairment or functional limitation restricts the activities needed to study at a postsecondary level or join the labour force and is expected to last for the student's expected life. Persistent or prolonged means the same restriction has lasted, or is expected to last, at least 12 months but not for life. Answering No to both lets you skip Parts 3 to 6 and sign Part 7, with Parts 1, 2 and 7 complete.

Part 3: Nature of patient's disability

Check every category that fits: acquired brain injury, ADD/ADHD, autism spectrum disorder, chronic health conditions such as Crohn's disease, epilepsy, chronic pain or a heart condition, deaf or hard of hearing, functional or mobility impairments, learning disability, mental health impairment, visual impairment, or another disability you name. For a learning disability, record whether a registered psychologist, psychological associate or neuropsychologist did a psycho-educational assessment, its most recent date, and whether a learning disability was confirmed.

Part 4: Mobility/movement and/or sensory impacts

Select the physical and sensory impacts that apply, such as ambulation, standing, sitting, stair climbing, lifting or reaching, dexterity, low vision after correction, legal blindness or hearing loss, then describe them. Part 6 asks for recommendations based on these functional limitations, so a specific description here backs up what you recommend later. Tick "No mobility/movement or sensory impacts" if none apply, so the part is not left blank.

Part 5: Cognitive and/or behavioural impacts

Select cognitive and behavioural impacts: attention and concentration, memory, verbal or written information processing, stress management, communication, organization and time management, social interactions or other, and describe them in the box. If none apply, tick "No cognitive or behavioural impacts". Avoid vague terms: Section B specifically warns against phrases such as "suggests", "is indicative of" and "undisclosed".

Part 6: Recommended accommodations or supports for postsecondary studies

Based on the student's functional limitations, tick each recommendation: reduced course load, specialized equipment or specialized services, specifying what is needed, or None. If approved, the BSWD and CSG-DSE may help pay for the services and equipment you list here, though funding isn't guaranteed. A blank Part 6 doesn't stop disability verification, but the student may not be eligible for the BSWD or CSG-DSE.

Part 7: Declaration of physician or regulated health care professional

Sign and date to certify that the information is accurate and, if you answered Yes in Part 2, that the student has the functional limitations and educational barriers indicated. For an electronic signature, use Fill & Sign or a Digital ID in Adobe Reader or another PDF program. Give the completed form and any attachments back to the student, who submits it.

Common mistakes

  • Leaving any field in Parts 1 to 5 or Part 7 blank: OSAP then won't consider the student to have a permanent or persistent or prolonged disability.
  • Leaving Part 6 blank: the form warns the student may then not be eligible for the BSWD or CSG-DSE disability grants.
  • Omitting the office stamp or signed letterhead, or leaving the licence chart incomplete; the form says it will not be accepted.
  • Vague phrases such as "suggests", "is indicative of" or "undisclosed" instead of clear statements about the disability, limitations and educational barriers.
  • Relying on an Individual Education Plan to document a learning disability; OSAP doesn't accept one, and an old or missing psycho-educational assessment may prompt requests for more documentation.

Submitting and payment

How to submit it
Return the form to the student, who uploads it in their OSAP account or sends it to the financial aid office of their Ontario private school, or by mail to the ministry's Student Financial Assistance Branch if the school is outside Ontario. It must be received no later than 40 days before the study period ends (5 days after it ends for micro-credentials).
Who pays
No fee or billing code appears on the form, and OSAP's 2026-27 editions don't say who pays; the 2024-25 public-school edition told students they were responsible for any fee their provider charged.

Good to know

  • A blank anywhere in Parts 1 to 5 or 7 means the student won't be considered disabled for OSAP; a blank Part 6 may cost them BSWD and CSG-DSE eligibility.
  • The separate BSWD/CSG-DSE application must be received 60 days before the end of the study period, with all supporting documentation due 40 days before the end.
  • Each BSWD/CSG-DSE request for a service or piece of equipment is judged against the student's specific functional limitations, using information from the health care provider or accessibility office.
  • Reduced course load is one of the Part 6 options; students with a verified disability can be treated as full-time at 40% of a full course load, instead of the usual 60%.

Frequently asked questions

How is this different from the public-school OSAP disability form?

This edition is for students at Ontario private schools or OSAP-approved schools outside Ontario. It adds Part 6, recommended accommodations or supports, which moves the declaration to Part 7. Students at schools outside Ontario send it to the ministry instead of a financial aid office. Students at Ontario public postsecondary schools use the public-institution edition, which ends at Part 6.

What should I recommend in Part 6?

Base your recommendations on the student's disability-related functional limitations, ticking all that apply: reduced course load, specialized equipment or specialized services, specifying what is needed in each case, or None. If the student applies and is approved, the BSWD and CSG-DSE may help fund the recommended services and equipment, but funding is not guaranteed.

Can I put my recommendations in a letter instead?

For the BSWD/CSG-DSE application, a written recommendation from you or a school disability advisor for the specific services or equipment requested is accepted when the recommendations weren't given on the verification form. On the form itself, Part 6 includes a None option, and leaving the part blank may make the student ineligible for those grants.

Is this OSAP form available in French?

Yes. OSAP publishes the 2026-27 private-institution edition in French as well as English, titled "Formulaire de vérification de l'invalidité : Étudiants qui fréquentent des établissements privés ou des établissements approuvés par le RAFEO à l'extérieur de l'Ontario". The public-institution edition also has a French version.
