ESDC SDE0061 MED: Severe Permanent Disability Report

Student and apprentice loan borrowers applying for the Severe Permanent Disability Benefit need this nine-page medical report from a licensed physician or nurse practitioner. If medical adjudicators find them eligible, their Canada Student Loans, applicable provincial student loans and Canada Apprentice Loans are cancelled. The report asks whether a permanent disability prevents substantially gainful work for life, with diagnoses, treatments and a prognosis.

Who completes it

A licensed physician or nurse practitioner completes Sections B to E: nature and history of the disability, treatments and medications, prognosis, and identification and signature. The patient completes Section A and the separate SPDB application.

Submitting and payment

How to submit it
Give the original report and supporting documents to the patient, or mail them to the Canada Student Financial Assistance Program and tell the patient; do not fax or email it, and keep a copy for the chart.
Who pays
The patient is responsible for any fees to complete the report, and the CSFA Program does not reimburse them.

Good to know

  • The application and medical report must be completed, signed and dated within the last 12 months, and only the original medical report is assessed.
  • The form asks you to be precise and avoid words such as possible, probably, likely or unknown when describing the disability and the prognosis.
  • Substantially gainful means work paying at least the maximum annual CPP disability pension, so eligibility for another disability benefit does not mean eligibility for this one.
  • Question 13 applies only to Ontario Student Loans and Ontario Micro-credential Student Loans: whether the patient will ever be able to work or pursue post-secondary education.

Frequently asked questions

Who can complete the Severe Permanent Disability Benefit Medical Report?

Only a licensed physician or nurse practitioner. Sections B, C and D must be completed by one of them, and Section E asks for your provincial or territorial licence number, type of practice (general practice, nurse practitioner or specialist), stamp, signature and date. The patient fills in Section A.

Can the SPDB medical report be faxed?

No. The instructions say not to fax or email the report. You can give it and any supporting documents to the patient, or mail the original directly to the Canada Student Financial Assistance Program and let the patient know. Only the original report is assessed, so keep a copy for your file.

What happens if the benefit is approved?

The borrower's obligation to repay Canada Student Loans, applicable provincial student loans and Canada Apprentice Loans is cancelled. ESDC's benefit page adds that they would then not be eligible for any additional Canada Student Grants or Loans, and that receiving the benefit is neither automatic nor guaranteed.
