ESDC SDE0083: RAP-D Verification of Disability

First-time applicants to the Repayment Assistance Plan for Borrowers with Disabilities (RAP-D) who have not received a PD or PPD grant use this four-page form to prove a permanent, or persistent or prolonged, disability. It covers Canada Student Loans and Canada Apprentice Loans. A licensed medical practitioner completes Part 2 on the nature, expected duration and functional limitations of the disability.

Who completes it

A licensed medical practitioner registered where the assessment is done, such as a physician, nurse practitioner, audiologist, optometrist or psychologist, completes Part 2. The applicant completes and signs Part 1.

Submitting and payment

How to submit it
Return the completed questionnaire to the applicant or mail it to the Canada Student Financial Assistance Program; the National Student Loans Service Centre says the form can also be submitted electronically through its website.
Who pays
Part 1 states that any costs charged to complete the form, such as doctors' fees, are the applicant's responsibility.

Good to know

  • Instead of Part 2, the applicant can submit documents proving receipt of federal or provincial disability assistance with a completed and signed Part 1.
  • Part 2 asks for the nature of the disability, date of onset, and expected duration: less than 12 months, 12 months or more, or the remainder of the applicant's life.
  • Limitations are grouped as mobility or motor skills, sensory or communication, work tolerance, cognitive or perceptual, and mental; the questions on devices and medications are optional.
  • RAP-D can lower monthly payments further to help with disability-related expenses, and borrowers apply through the regular RAP application in their NSLSC online account.

Frequently asked questions

Who needs the RAP-D Verification of Disability form?

According to the National Student Loans Service Centre, borrowers applying for RAP-D for the first time who have not received any PD or PPD grants must complete it to confirm eligibility. Applicants can attach proof of federal or provincial disability assistance instead of having a practitioner complete Part 2.

Which practitioners can complete Part 2 of SDE0083?

An appropriate medical professional registered to practise in the jurisdiction where the assessment is done, such as a physician, nurse practitioner, audiologist, optometrist or psychologist. You certify that you are a legally qualified licensed medical practitioner, give your licence number and type of practice, and stamp, sign and date the form.

What if the applicant does not sign Part 1?

The form says it will not be processed if the applicant fails to sign and date it. An authorized representative may sign instead with proof of authority, such as a power of attorney, trustee or guardian appointment, and applicants are told to keep receipts for disability-related expenses.
