Alberta Student Aid Disability Verification Form

Alberta Student Aid uses this four-page form as one of the criteria for federal and provincial disability grant funding for post-secondary students with a permanent, or persistent or prolonged, disability. The medical assessor confirms the disability type and status, describes functional limitations in an educational setting and recommends accommodations, such as a reduced course load of 40 to 59%. Students with a learning disability send a psycho-educational assessment instead.

Who completes it

The medical assessor completes Sections 4 to 7, and the form names the right assessor for each disability type, such as an audiologist or physician for hearing and a physician, registered psychologist or psychiatrist for ADHD. The student completes Section 1.

Submitting and payment

How to submit it
Students send documents to Alberta Student Aid electronically through Upload Documents in their Alberta Student Aid account, which needs a verified Alberta.ca account; school staff cannot upload on a student's behalf.
Who pays
Students pay for the assessment upfront and can be reimbursed once an eligible disability is confirmed, if they attach a paid receipt and the assessment was used in the verification.

Good to know

  • The medical professional must initial in ink at the bottom of pages 1 to 3 and complete every required field in Section 7, including specialty, registration certificate number and office stamp or address.
  • Choose only one status for each disability: permanent, expected to last for life, or persistent or prolonged, lasting or expected to last at least 12 months but not for life.
  • Section 5 covers mobility, cognitive and behavioural impacts in an educational setting, their frequency and severity, and any medication side effects that may affect participation.
  • A learning disability needs a complete psycho-educational assessment less than 5 years old, or done at 18 or older, stating a DSM diagnosis, instead of the rest of this form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete the Alberta Student Aid disability verification form?

It depends on the disability. Mobility needs a physician or medical specialist; hearing, an audiologist or physician; vision, an optometrist, ophthalmologist or physician; brain injury, a physician or neuropsychologist; ADHD, autism and psychiatric conditions, a physician, registered psychologist or psychiatrist; and speech, a physician or speech-language pathologist.

Can the student be reimbursed for the assessment fee?

Yes, in some cases. Students pay upfront and may be reimbursed once an eligible disability is confirmed, by attaching a paid-in-full receipt with the assessment. Only assessments used during the verification qualify, none older than six months before the study period starts, and costs covered by insurance or other sources are excluded.

What accommodations can the assessor recommend?

Section 6 lets the assessor recommend a reduced course load of 40 to 59% of a full-time load, services such as tutoring, note-taking, alternate formats or sign-language interpreting, and equipment or assistive technology such as a laptop, specialized software, a low-glare monitor or a sit/stand desk.
