New Brunswick Student Aid Disability Verification Form

New Brunswick Student Financial Services uses this 15-page form to verify a permanent, or persistent or prolonged, disability for the Canada Student Grant for Students with Disabilities and related funding. A qualified medical assessor completes Section B on disability status, type, impacts on post-secondary studies and recommended accommodations, then returns the form to the student. It must arrive six weeks before the study period ends.

Who completes it

A qualified medical assessor completes all of Section B, with the profession set by disability type, for example a physician or nurse practitioner for mobility or cognitive impairment, an audiologist for hearing and a speech-language pathologist for speech. The student completes Section A, including a two-page declaration and consent.

Submitting and payment

How to submit it
The assessor returns the form to the patient, who submits it through the Upload a Document service on New Brunswick's student aid website, by fax or by mail, at least six weeks before the study period ends.
Who pays
The patient pays any fee, but it may be reimbursed through the CSG-DSE if the assessment was done no earlier than six months before the study period and the disability is approved.

Good to know

  • Section B forms completed electronically, or that appear to be completed by someone other than the medical assessor, are not accepted; initial every page and sign by hand.
  • Describe impacts specific to the student, with duration, frequency and effects, and avoid terms such as may, suggests or is indicative of; referring to the student's own statement is not accepted.
  • For visual impairment, criteria include visual acuity of 6/21 (20/70) or less in the better eye after correction, a visual field of 20 degrees or less, or a qualifying progressive eye disease.
  • A learning disability needs a psychoeducational assessment by a registered psychologist, done within five years or at 18 or older, in place of Section B.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can Section B of New Brunswick's form be filled in electronically?

No. Section B states that forms completed electronically or that appear to be completed by someone other than the medical assessor will not be accepted. The assessor initials the bottom of each page and gives a wet signature on the last page, and no part may be completed by the patient, parent or partner.

When is the New Brunswick disability verification form due?

Student Financial Services must receive it no later than six weeks before the end of the study period, because funds cannot be released after the study period ends. Incomplete forms are not processed and trigger a letter requesting proper completion, which delays funding.

Does the student need a new form every year?

Normally no. The form says it usually needs to be completed only once, unless the student develops another disability that requires additional accommodations, services or equipment. A student who is providing additional information, or whose situation has changed, checks that box in Section A.
