NWT Form G (NWT9008): Disability Assessment Form

NWT Student Financial Assistance uses Form G, a bilingual English and French form, to determine eligibility for the NWT Study Grant(s) for Students with Disabilities. The grant helps with exceptional education costs such as a tutor, interpreter, note-taker, attendant care or special equipment. A qualified practitioner, such as a medical doctor, confirms the disability type, date of diagnosis and status, and the education barriers it creates.

Who completes it

A certifying medical professional, such as a medical doctor, completes Section 2 in full. The student completes Section 1 first and consents to release of the information to the Student Financial Assistance Program.

Submitting and payment

How to submit it
The practitioner returns the form to the student to submit, or submits it on the student's behalf through sft.gov.nt.ca, the return channel the form names.
Who pays
Any fees charged for completing the form are the student's responsibility and are not reimbursed by the Department of Education, Culture and Employment.

Good to know

  • All sections are mandatory; place a dash or line through any boxes that do not apply.
  • The status choices are permanent, persistent or prolonged (expected to last at least 12 months), or temporary.
  • Disability types listed include mobility, hearing, visual, speech, pervasive developmental disorder, acquired brain injury, ADD/ADHD, and psychiatric or psychological learning, which calls for further explanation.
  • Item 5 asks you to identify all disability-related education barriers and how they prevent participation in post-secondary studies, including the duration and effects of the limitations.
