StudentAidNL Verification of Eligibility (Disabilities)

StudentAidNL uses this three-page form as one of the criteria for the Canada Student Grant for Students with Disabilities under the Canada-Newfoundland and Labrador integrated program. The medical or educational assessor confirms whether the student has a permanent or persistent/prolonged disability, gives its nature and primary diagnosis, and lists functional limitations affecting post-secondary studies and the labour force, then returns the form to the student.

Who completes it

A qualified medical or educational assessor completes Section C, Determining Eligibility, and Section D, Certification, with their professional designation and registration number. The student completes Section A, personal information and consent, and Section B on learning disabilities.

Submitting and payment

How to submit it
The form prints three ways to submit it to the Student Financial Services Division: email, fax or mail.

Good to know

  • If the answer to question 1 is no, the form tells the assessor to stop there because the student is not eligible.
  • StudentAidNL may use the list of education limitations to decide eligibility for the CSG-DSE; with no limitations for post-secondary education, the student is not eligible for that grant.
  • Students with only a learning disability skip Sections C and D and submit a psycho-educational assessment completed within the last 5 years or at 18 or older.
  • According to the form, students with a disability count as full-time at 40% or more of a full course load and may get extra time to finish their program.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Which sections of the StudentAidNL form does the assessor complete?

Section C, Determining Eligibility, and Section D, Certification of Medical/Educational Assessor. Section C asks whether the disability is permanent or persistent/prolonged, its nature and primary diagnosis, and the functional limitations for post-secondary education and the labour force; Section D needs your designation, registration number, stamp and signature.

What if the student has a learning disability and another disability?

StudentAidNL needs both: the psycho-educational assessment for the learning disability and this form completed by a medical professional for the other disability. A student with a learning disability only completes Sections A and B and attaches the assessment instead of having an assessor complete the rest.

Does the StudentAidNL form have to be completed every year?

StudentAidNL's disabilities page says documentation is normally required only once, unless the student has another diagnosis that requires additional accommodations, services or equipment. Funding for disability-related services and equipment needs a separate application, which the school completes.
