Nunavut FANS Disability Assessment Form (A-207)

Financial Assistance for Nunavut Students (FANS) uses this three-page form to certify a permanent disability for the Nunavut Study Grant for Students with Disabilities. The grant helps with exceptional education costs such as a tutor, interpreter, note-taker, attendant care or special equipment. The certifying medical professional records the diagnosis, whether the disability is permanent, and its effect on post-secondary studies, including course load.

Who completes it

The certifying medical professional completes Sections B and C fully, including their office stamp and signature. The student completes Section A with their personal details and consent to release information to FANS.

Submitting and payment

How to submit it
The medical professional returns the completed form to the student, who emails it to FANS, using one address for Inuit enrolled under the Nunavut Agreement and another for other students.
Who pays
Any fees charged for completing the form are the student's responsibility and are not reimbursed by the Department of Education.

  • Section C asks whether the student can study at the regular course load of 60% of a full course load, or whether you suggest a reduced level of 40%.
  • You also state whether the student needs extra educational aids related to the disability, describe the equipment, and identify all disability-related education barriers.
  • The form, last revised February 2024, carries Inuktitut and French department headings alongside English, and the Government of Nunavut lists it under the code A-207.
