IRCC CIT 0547: Medical Opinion for Citizenship Waivers

A medical practitioner licensed in Canada completes this form when a patient applying for Canadian citizenship asks IRCC to waive the language and knowledge (citizenship test) requirements, the oath, or both, because of a severe and prolonged medical condition. Eligibility turns on the severity, duration (1 year or more) and functional impact of the impairment, not on the diagnosis itself.

Who completes it

The attending practitioner (a GP or family physician, other physician specialist, psychologist or nurse practitioner) completes Section A (language and knowledge), Section B (oath), Section C (general comments) and the identification and certification block. The patient or their representative signs the authorization to release the information to IRCC.

Submitting and payment

How to submit it
The patient submits it to IRCC to support the waiver request, alongside the Waiver Request Form (CIT 0116) for the language and knowledge requirements and the citizenship application.
Who pays
The form states that the patient is responsible for any fee the practitioner's office charges to complete it.

Good to know

  • Language and knowledge requirements apply to applicants aged 18 to 54; an oath waiver can be considered for adults with a mental disability and, on compassionate grounds, for minors 14 to 17.
  • For cognitive, psychiatric or psychological conditions, attach the latest test results, specialist reports and psychologist or psychiatrist referral notes; past or resolved conditions should not be listed.
  • GPs and family physicians supporting an oath waiver are asked whether a specialist in mental disability has examined the patient, and to attach that report or explain why not.
  • IRCC relies on the opinion to decide the waiver, and the form warns that the practitioner may be cross-examined in Federal Court proceedings.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Who can complete CIT 0547?

A medical practitioner licensed to practise in Canada. The certification block lets the attending practitioner identify as a general practice or family physician (CCFP), other physician specialist, psychologist or nurse practitioner, and asks when they became the patient's practitioner and how many visits occurred in the past 12 months.

Is CIT 0547 available in French?

Yes. IRCC publishes it in English and French (CIT 0547 F), last updated May 2024. IRCC asks users to download the fillable PDF to a computer and open it in Adobe Acrobat Reader version 10 or higher, because it may not open on tablets or phones.

What does IRCC look at in the medical opinion?

Eligibility depends on the severity of the impairment, its duration and its impact on functional abilities, not on the type of condition. The form asks for the diagnosis with onset dates, the basis of the assessment, how the condition prevents basic communication or test study, treatment, expected duration and effects on daily living.
