IRCC IMM 1017: Medical Report, Client Biodata & Summary

IRCC uses this single standard form for immigration medical examinations (IMEs) in all immigration categories. It records the client's biodata and immigration details, the IMM type (excessive demand exempt, non-EDE or refugee overseas), the IME grade and the panel physician's declaration. Only IRCC-designated panel physicians complete it, almost always in eMedical rather than on paper.

Who completes it

The panel physician grades the IME (A: no significant abnormal history or findings; B: significant findings, with mandatory comments), confirms the identity document was sighted, notes any identity concerns and signs the declaration with their panel physician number. The applicant signs the Client Declaration and Notice (IMM 5743), without which the IME cannot be submitted.

Submitting and payment

How to submit it
Panel physicians in countries where eMedical is implemented must complete and transmit IMEs in eMedical; paper submissions to the Regional Medical Office are allowed only with the RMO's approval.
Who pays
The client pays the panel physician directly at the exam and the fee is not refundable, even if the application is refused; the Interim Federal Health Program may cover the exam for eligible clients.

Good to know

  • Clients who receive a paper instruction letter must be examined within 30 days of its date and bring the IMM 1017, government photo ID and any existing medical reports.
  • Non-EDE applicants are assessed for danger to public health or safety and excessive demand on health and social services; EDE applicants, including refugees, are not assessed for excessive demand.
  • On paper IMEs, each attached photo must be signed by the panel physician to confirm identity, and photos must be taken within six months of the exam.
  • Upfront medicals for students, workers and visitors use IMM 1017B UPFRONT, or an eMedical information sheet, as the client's proof of completion for their visa application.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can a family doctor complete the IMM 1017?

No. Only IRCC-designated panel physicians can perform immigration medical examinations and complete the IMM 1017. Clients find a panel physician on IRCC's list; reports from their own physicians about existing conditions can be brought to the appointment and may help reduce processing time.

What do grades A and B mean on the IMM 1017?

Grade A means no significant abnormal history or abnormal findings, and no comments are permitted. Grade B means significant abnormal history or findings, and comments are mandatory. In eMedical a system-assigned B grade cannot be changed to A, and IMEs for clients with abnormal HIV serology are graded B.

How long is an immigration medical exam valid?

IRCC's Panel Members' Guide refers to the 12-month validity of the medical and tells upfront applicants to submit their visa application with proof of the exam as soon as possible after the appointment, so the application can be processed within that period.
