VAC 743: Psychological Progress Report

Veterans Affairs Canada asks the treating psychologist or mental health provider for this progress report on psychological treatment it funds, to support the treatment plan, authorization of further services and case management. The report covers a set period: sessions held, missed appointments, conditions treated per the current DSM, test interpretation, clinical objectives and interventions, progress made, and whether psychotherapy should continue.

Who completes it

The treating psychologist or mental health provider registered with Medavie Blue Cross completes the whole report and signs it with their professional title, professional corporation, licence number and Blue Cross number. The client is identified by name, date of birth and any service or RCMP regimental number.

Good to know

  • State the period the report covers, the number and length of sessions, and how many times the client failed to attend or cancelled within 24 hours.
  • The form asks whether any factors prevented progress and whether you had to modify the treatment plan, its interventions or clinical objectives, during the period.
  • If you recommend continuing psychotherapy, give the clinical objectives, recommended frequency and estimated duration; there is also space for further recommendations such as ongoing risk issues.
  • The current edition is dated 2017-04, is fillable, and is published in French as Rapport d'évolution de la psychothérapie.
