VAC 1319: Report by Medical Examiner

Veterans Affairs Canada uses this report from an examining physician to decide eligibility for the War Veterans Allowance and benefits under the Civilian War-Related Benefits Act. The examiner records complaints, history, physical findings, a functional and psychosocial assessment and diagnoses, then gives an opinion on employability, prognosis and potential for retraining. Because the allowance depends on this medical information, the form asks for early transmission.

Who completes it

The examining physician completes sections 1 to 9, states the date of examination and whether they are the patient's own physician, and signs. The top of the form identifies the client by name, VAC file number, mailing address and any service or RCMP regimental number.

Submitting and payment

How to submit it
The completed report goes back to VAC without the address cover sheet, which the form says is for address purposes only, and VAC asks for it to be sent early.

Good to know

  • Section 4 special studies, such as x-ray, serology, urinalysis or EKG, are done only when authorized by the VAC Medical Adviser or District Authority; results can follow later if a tentative assessment is possible.
  • List multiple diagnoses in order of significance for employability, and support the employability opinion in section 7 with a narrative summary in section 8.
  • Section 5 asks for general appearance, speech and affect as they might impress a prospective employer, plus a psychosocial assessment; append any psychiatric or psychological report obtained.
  • Section 7 records unrestricted or restricted employability, whether the client is probably permanently unemployable, prognosis with adequate treatment, and retraining potential: good, moderate or not feasible.
