Health Canada SAP Form C: Follow-Up Information

After a practitioner uses a drug obtained through Health Canada's Special Access Program, the Food and Drug Regulations require a report on the results, including adverse reactions. Form C is that follow-up, used when a drug was requested for a patient only once. Health Canada uses these reports when processing requests and to help detect product-related safety issues.

Who completes it

The practitioner who requested and received the drug completes the whole form: practitioner details, patient initials, drug use, treatment response, adverse reactions and the signed attestation with licence or college number.

Submitting and payment

How to submit it
Fax the completed, signed and dated form to the Special Access Program without a cover sheet, quoting the original SAP request number if available.

Good to know

  • If the patient had a serious and/or unexpected adverse reaction, the form directs the practitioner to also complete a CIOMS adverse reaction form.
  • Serious unexpected reactions must be reported to the SAP within 15 days of becoming aware of them, and fatal or life-threatening ones within seven days.
  • To continue treatment, a follow-up report on the outcome is required before requesting more drug, using Form A for the repeat request.
  • Form C is a five-page, legal-size (8.5 × 14) fillable PDF; Health Canada also offers a French version and a Word version.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

When is SAP Form C required?

Health Canada's guidance says Form C is used when a drug was requested for a patient only once, meaning no further requests will be made for that patient and drug. If treatment continues, the outcome is reported before more drug is requested through a repeat Form A.

What does SAP Form C ask about treatment results?

It asks whether treatment started, the quantity given and dates of administration, any dose changes, the treatment goal, whether the response was none, partial or complete with a description, concomitant medications or therapies, and whether any serious or unexpected adverse reactions occurred.

What counts as a serious and unexpected adverse reaction on Form C?

The form defines it as a serious adverse drug reaction that is not identified in nature, severity or frequency in the risk information on the drug's label. If one occurred, the practitioner also completes a CIOMS adverse reaction form and should stay vigilant, since most SAP drugs are still in clinical development.
