Transport Canada 26-0010: Aviation Medical Exam Report

A Civil Aviation Medical Examiner (CAME) records an aviation medical examination on this report, which Transport Canada uses to issue or renew medical certificates for pilots, air traffic controllers and other licence and permit holders. The applicant completes a separate Statement of Applicant (26-0853). CAMEs submit their findings through Transport Canada's online Electronic Medical Examination Report (eMER).

Who completes it

Only a CAME designated by Transport Canada completes the examination report, assessing the person as fit, unfit or needing more tests. The applicant fills in the Statement of Applicant (26-0853) separately.

Submitting and payment

How to submit it
The CAME submits the findings through the eMER, even when the examination is incomplete, along with any specialist reports Transport Canada asked for.

Good to know

  • Submit through the eMER even if the examination could not be completed, for example when a telemedicine assessment proves insufficient and an in-person exam is scheduled.
  • If special medical reports are a condition of renewal, the CAME reviews them before granting the renewal and submits them through the eMER.
  • Telemedicine renewals also go through the eMER, and a holder found fit receives a Civil Aviation Fit Certification (26-0862A).
  • Transport Canada's Privacy Notice for Aviation Medical Examinations (26-0886) goes with the exam; the report is version 2602-10, in English and French.

Frequently asked questions

Who can complete Transport Canada form 26-0010?

Only a Civil Aviation Medical Examiner designated by Transport Canada; applicants find one with Transport Canada's CAME search tool. A family physician who is not a CAME cannot complete it, though any physician or optometrist may report a condition relevant to aviation safety on form 26-0011.

How do CAMEs submit the aviation medical examination report?

Through Transport Canada's online Electronic Medical Examination Report (eMER). Transport Canada says CAMEs must submit their findings there even when an examination is incomplete. The eMER records whether the applicant or renewal holder is fit, unfit or needs more tests, and CAMEs get access after their appointment.

What does the applicant complete for an aviation medical?

The Statement of Applicant (26-0853), a separate form from the CAME's examination report. Transport Canada also provides a Privacy Notice for Aviation Medical Examinations (26-0886). The CAME then examines the applicant, reviews any specialist reports Transport Canada asked for, and records the findings in the eMER.
