TC Form 26-0297: Category 4 Medical Declaration

Applicants for aviation permits that need a Category 4 medical certificate declare their own medical fitness on this form. For a student pilot permit for aeroplanes or a recreational pilot permit, a family physician also countersigns Part B. If the applicant has had any condition listed in Part B, the declaration is not enough and a Civil Aviation Medical Examiner must examine them instead.

Who completes it

The applicant completes the declaration. For a student pilot permit (aeroplane) or a recreational pilot permit, a family physician countersigns Part B, and that physician does not need to be a CAME.

Submitting and payment

How to submit it
The applicant emails the completed declaration to a Transport Canada Centre.

Good to know

  • Any family physician can countersign Part B; the countersigning physician does not have to be a Civil Aviation Medical Examiner.
  • If the applicant has had any condition listed in Part B, they need a CAME examination rather than this self-declaration.
  • The physician's countersignature is required for the student pilot permit for aeroplanes and for the recreational pilot permit.
  • The current version is 1803-08, a PDF in English and French downloadable from Transport Canada's forms catalogue.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Frequently asked questions

Can my family doctor sign the Category 4 medical declaration?

Yes. Any family physician can countersign Part B of form 26-0297; the physician does not have to be a Civil Aviation Medical Examiner. The countersignature is needed for a student pilot permit for aeroplanes or a recreational pilot permit, and the applicant then emails the declaration to a Transport Canada Centre.

When is a CAME exam needed instead of form 26-0297?

When the applicant has had any of the conditions listed in Part B of the declaration. In that case the self-declaration route is closed, and the applicant needs an examination by a Civil Aviation Medical Examiner, who reports the findings to Transport Canada.

Is form 26-0297 available in French?

Yes, as the Déclaration médicale pour les licences et les permis nécessitant une catégorie 4 norme médicale. The English and French versions are both PDFs, version 1803-08, downloadable from Transport Canada's forms catalogue.
