VAC 922B: Request for Extension of Treatment

Veterans Affairs Canada pre-authorizes certain treatment services only up to an annual number of sessions set in its Benefit Grids. When a Veteran needs more, the treating provider uses this form to show, with objective measures, that treatment has worked and is reasonably expected to keep working. Medavie Blue Cross's Medical Authorization Centre receives the request, and VAC decides whether to pay for additional sessions.

Who completes it

The treating health professional completes the provider, treatment and recommendation sections and signs, giving their Medavie Blue Cross provider number and professional title. The Veteran, or their representative, completes the identification section and signs the certification.

Submitting and payment

How to submit it
Send it to Medavie Blue Cross's Medical Authorization Centre by mail or fax when five sessions remain, or with a pre-authorization request through the Provider Portal (e-Pay).
Who pays
VAC pays a report fee for completing the form at your usual and customary rate, up to the maximum in the Benefit Grids, but does not pay for incomplete forms.

Good to know

  • Describe the condition at the start of treatment with objective measures such as range of motion, strength testing, pain scales or standardized tools, then give a current measure for comparison.
  • Goals must be specific, measurable and attainable, for example pain decreasing from 5 to 3 or sitting for 6 hours instead of 3, with the modalities used for each.
  • A Veteran's self-assessment alone is not enough, and requests without objective evidence of effectiveness are rejected; for maintenance treatment, show how it keeps the Veteran at their current state.
  • If the last five sessions will be used within 30 days, the form asks you to contact the Medical Authorization Centre; prescription requirements must still be met each year.
