Physician, specialist, chiropractic, physiotherapy, psychology and opioid management reports for WCB-Alberta claims, and which ones go through myWCB.
WCB-Alberta is Alberta's workers' compensation board. Non-specialist physicians complete the Physician first report (C050) at every first visit for a work injury. The Physician progress report (C151) follows when something relevant changes. Each combines the report and the invoice. Chiropractors start with the Chiro first report (C056) and physiotherapists with the Physiotherapy services first report (C533). Specialists, psychologists, social workers and other providers have their own reports.
The C050 must be reported electronically, through myWCB Electronic Injury Reporting or accredited medical software, and the C151 is filed the same way; paper copies can only be ordered. Fees are highest for reports received by 10:00 MT the next business day. Specialists bill on the Medical Consultation Report (C568A) in Electronic Injury Reporting. WCB-authorized chiropractors and physiotherapy clinics report in myWCB, while others use the PDFs. Vero Documents fills PDF reports such as C056 and C533 from the visit note and the chart.

Electronically. WCB-Alberta requires the C050 through myWCB Electronic Injury Reporting or accredited medical software, and paper copies can only be ordered, not downloaded. Section 34 of the WCA requires a report within two days of the first visit when disability beyond the day of the accident is likely. The fee is highest when the report arrives by 10:00 MT the next business day, lower when on time and lowest when late.
When relevant changes have occurred since the first report. File the Physician progress report (C151) as a follow-up report in myWCB or from an accredited EMR; it counts as on time if received within four business days. Separately, you must tell WCB-Alberta when the worker can return to work, no later than three days after that date.
Chiropractors send the Chiro first report (C056) within two business days of the initial assessment. The progress/discharge report (C352) follows at the end of week 3 and within two business days of discharge. Physiotherapists send the first report (C533) and a progress or discharge report (C534). WCB-authorized providers report through myWCB; non-authorized providers use the PDFs.
A psychologist or clinical or master's-level social worker who suspects a client's mental health condition is work-related, or any regulated professional with a duty to report. File it within 48 hours of suspecting the link, once per client. Contracted community psychology providers report through myWCB, then use the initial (C851), progress (C852) and discharge (C853) counselling reports. Others use the Word form.
