Contracted psychologists and master's-level social workers use this report after the final counselling sessions with a referred worker. It updates the worker's psychological status and the psychosocial measures baselined in the initial report, summarizes the treatment provided, records the discharge date, number of no-shows and reason for discharge, and recommends any maintenance work.
The treating clinician types and signs the report on the C853 template; a supervising clinician or office staff cannot sign on their behalf.

This form is a Word template, not a PDF. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Promptly after the final session, because the fee depends on the business days between the final counselling session and report submission: within 7 business days you bill PPMR13A, and after that the late fee PPMR13B applies. If a worker stops attending, WCB-Alberta's progress report guide also directs you to complete a discharge report.
It asks for symptoms, objective findings and any working diagnosis, updated psychosocial measures against the initial baseline, progress on each treatment goal as a percentage, and return-to-work details. Those cover accommodations to schedule, hours, duties, safety-sensitive work and location, plus an estimated date for return to pre-accident work.
