Psychologists and master's-level social workers contracted with WCB-Alberta use this report after the first counselling session with a referred worker. It sets out the need for psychological services, the worker's current status, barriers to recovery and return to work, and a treatment plan with baseline psychosocial measures and goals, plus any questions the claim owner has raised.
The clinician who provided the counselling types and signs the report on the C851 template; a supervising clinician or office staff cannot sign it, though a digital signature is accepted.

Psychology initial counselling report - C851
This form is a Word template, not a PDF. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

The fee depends on timing. If WCB receives the report within 7 business days of the initial counselling session, you bill the timely initial report fee, PPMR11A; after 7 business days the late fee, PPMR11B, applies. Progress reports then follow monthly, unless the initial report went in on or after the 15th of the month.
It asks whether you have identified a working diagnosis or clinical impression and, if so, to describe it based on DSM-5-TR. It also asks for symptoms as the worker describes them, objective findings such as hygiene, daily living and affect, post-accident mental health treatment, and any prior mental health conditions and treatment.
