Acupuncturists use this one-page report to update WCB-Alberta on a worker's treatment, ticking progress when requesting more treatment and discharge when care is complete. It covers complaints, a pain or anxiety rating, how long the worker stays pain free after treatment, objective findings, positive outcomes the worker reports, complications, return-to-work status and the total number and dates of treatments.
The treating acupuncturist completes and signs the report, adding their WCB billing number and the name and address to whom the fee is payable.

Acupuncture progress/discharge report - M007A
If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Complete M007A with progress ticked and the number of treatments required, and fax it to WCB's PT consultant at the end of the third week or the fifth treatment, whichever comes first. WCB-Alberta's Acupuncture Reporting and Invoicing Guide says this report is required for any extension request.
Both. The same form serves as the progress report, used when requesting an extension, and as the discharge report when treatment is complete. They are billed differently: ACURF02 for a progress report and ACURF03 for a discharge report. The current version is M-007A REV JUN 2022.
