Chiropractors use this report to tell WCB-Alberta about a worker's injury after the initial assessment: how it happened, symptoms, objective findings, ICD-9 diagnostic codes, imaging, the treatment plan and work capabilities. WCB uses it to decide entitlement and plan return to work, and it is due within 2 business days of the initial assessment.
The treating chiropractor completes and signs the whole report, from worker details and the injury description to the job-requirements grid and estimated return-to-work date.

Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Within 2 business days of the initial assessment. WCB-Alberta's Chiropractic Contract Reference and Online Reporting Guide for 2025 to 2028 also sets the progress report on C352 at the end of the third week of treatment and the discharge report within 2 business days of discharge.
It depends on your status with WCB-Alberta. Chiropractors with a WCB contract report online through myWCB, where some fields pre-populate and reports are uploaded to the claim in real time. Non-authorized chiropractors use the C056 PDF, revised January 2024, and send it by fax or mail.
Page 2 asks whether the worker missed work beyond the date of accident and has returned to full or modified duties, then rates abilities such as sitting, standing, lifting, bending and driving as able, unable or limited. It ends with an estimated return to pre-accident work date or a note that permanent restrictions are anticipated.
