Specialists use this invoice-and-report in WCB-Alberta's electronic injury reporting for every service they provide to an injured worker, including consultations, follow-ups, surgery, anesthetic and diagnostic imaging claims. Any physician billing a supplementary report that WCB requested uses it too. The typewritten narrative report is attached, and no report fee is paid unless a report is submitted and invoiced.
Specialist physicians complete the invoice fields and attach their typewritten consultation, surgical or follow-up report; other physicians use it only to bill a supplementary report WCB has requested.

Medical Consultation Report (C568A)
WCB-Alberta takes this form through its online services or your EMR, so there is no PDF to fill. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

WCB-Alberta's Physician's Reference Guide lists the specialists' Medical Consultation Report as C568A and shows the medical invoice C-568 in its Appendix B. General practitioners use C568 for follow-up visits with no relevant change and for services that need no report, while specialists use C568A for all services and any physician uses it for supplementary reports.
When WCB receives it within 4 business days. The specialist fee schedule pays consultation reports (RF01E) and follow-up reports (RF03E) at three rates: same-day, on-time and late. The Physician's Reference Guide adds that no report fee is paid unless a report is submitted and invoiced, and that tray fees must be added because they are not automatic.
