Chiropractors treating a worker on a WCB-Alberta claim use this one-page report twice: as a progress report at the end of the third week of treatment and as a discharge report when care ends. It updates symptoms, objective findings, any change in diagnosis, the treatment plan and return-to-work status, and it is where a chiropractor asks to extend treatment.
The treating chiropractor completes and signs the whole report, including the job-requirements grid and estimated return-to-work date; the form also records who confirmed the job requirements.

Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

At the end of the third week of treatment, according to WCB-Alberta's Chiropractic Contract Reference and Online Reporting Guide for 2025 to 2028. The same form serves as the discharge report, due within 2 business days of discharge. The first report, C056, is due within 2 business days of the initial assessment.
Contracted chiropractors can use the online Chiropractor Finalize Treatment Report (C704) when the worker needs to be discharged and there is no new information about progress or status. WCB-Alberta does not pay for C704, so a complete discharge report on C352 is the payable option.
It asks for current symptoms and objective findings, whether the diagnosis has changed, up to three ICD-9 diagnostic codes, and the part of body, side of body and nature of injury. When reporting online through myWCB, the previous diagnosis and codes may pre-populate from earlier reports.
