Acupuncturists send this one-page report to WCB-Alberta after examining a worker referred for a work-related injury. It records the referring physician and referral date, diagnosis, complaints, a 0 to 10 pain rating (or anxiety for a psychological injury), acute or chronic objective findings, the treatment provided and the worker's return-to-work status.
The treating acupuncturist completes and signs the report, with the clinic name, WCB billing number and the name and address to whom the fee is payable.

Acupuncture first report - M007
Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Within 2 business days of completing the initial assessment, according to WCB-Alberta's Acupuncture Reporting and Invoicing Guide for 2025 to 2028. The progress/discharge report (M007A) follows within 2 business days of the seventh treatment, or earlier when you need to request an extension.
Not for approved services. Section 86 of the Workers' Compensation Act says no part of the cost of medical aid is payable by the worker, so approved treatment is billed to WCB-Alberta, not the worker or employer. They can pay privately only for services WCB has said are not payable or supported.
