Denturists use this one-page report when a worker's dentures or other prostheses are damaged in a work accident covered by WCB-Alberta. Completed at the first examination, it records the history of injury, damage to the prosthesis, emergency treatment, further treatment needed, pre-existing conditions and any specialist referral, and goes to WCB with a cost breakdown of the proposed treatment.
The examining denturist completes and signs the whole report, adding their WCB billing number and the name and address to whom the fee is payable.

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Within 2 business days of the examination, together with a cost breakdown of the proposed treatment on a standard dental claim form. Section 34(1) of the Workers' Compensation Act requires a report within two business days of first attending an injured worker, and WCB can request further reports at any time.
It can. Under section 79 of the Workers' Compensation Act, WCB may pay to repair or replace dentures lost, damaged or destroyed in an accident, regardless of the accident date. Treatment beyond the examination, treatment plan and emergency pain relief needs pre-authorization, and coverage is limited to restoring function to the pre-accident state.
