This three-page report gives WCB-Alberta a full update on a worker's physiotherapy: symptoms, objective findings, range of motion and strength, any change in diagnosis, interventions used, patient engagement, barriers and return-to-work status. Non-contracted physiotherapists file it at week 4 of treatment (week 8 for fracture or surgery), every three weeks during phase 2, at discharge, and to request a treatment extension.
The treating physiotherapist completes and signs the whole report, including whether the patient agrees with the return-to-work details; other disciplines cannot complete it.

Physiotherapy services comprehensive progress/discharge report - C534
Upload the fillable PDF once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Submit a comprehensive progress report with the extension question answered yes, faxed directly to the WCB physiotherapy consultant. Extensions apply when the worker is not fit to work and a return-to-work assessment is not appropriate. Interim visits, at most once a week for up to four weeks, can continue while you wait for a decision.
Within 2 business days of the end of week 4 of treatment under a standard authorization, or week 8 for fracture and surgery injuries. During phase 2 treatment after the initial authorization, WCB-Alberta requires a comprehensive progress report every three weeks until discharge, then a discharge report once treatment ends.
