WCB Alberta C533: Physiotherapy First Report

Physiotherapists send this report to WCB-Alberta after assessing a worker with a work-related injury. Its three pages cover how the injury happened, diagnosis and ICD-9 codes, range of motion and strength, pain, neurological and gait findings, the treatment plan, recovery barriers and the worker's ability to sit, stand, lift and drive. It is due within 2 business days of the assessment.

Who completes it

The assessing physiotherapist completes and signs the whole report; WCB-Alberta states these reports cannot be completed by disciplines other than physiotherapy.

Submitting and payment

How to submit it
Physiotherapy clinics contracted with WCB-Alberta report online through myWCB; non-contracted physiotherapists fax or mail the PDF within 2 business days of the assessment.
Who pays
For non-contracted physiotherapists, WCB-Alberta pays the assessment report as fee code RPT01, separate from the assessment visit (07.38AA), and reports must be invoiced because they are not paid automatically.

Good to know

  • Standard authorization covers up to 8 visits within 4 consecutive weeks; surgical and fracture injuries get up to 16 visits over 8 consecutive calendar weeks.
  • Tell the worker they are responsible for payment if physiotherapy is not authorized, and for verbal authorization record the date, time, services and approver's name.
  • WCB expects specific objective information such as range of motion, strength, pain on a visual analogue scale, gait aids, sit/stand/walk tolerances and functional abilities.
  • State fitness for work in every report, including potential for modified or alternate work, even if modified work is unavailable or the worker is not job-attached.

Frequently asked questions

When is the WCB Alberta physiotherapy first report due?

Within 2 business days of the date of the assessment. After that, the abbreviated progress report is due 2 business days after the end of week 2 (week 4 for fracture or surgery injuries), and the comprehensive progress report 2 business days after the end of week 4 (week 8 for fracture or surgery).

Is C533 the same as the Physical Therapy Assessment Report?

Yes. WCB-Alberta's older Physical Therapy Reporting Guide (C-510) called the first report the Physical Therapy Assessment Report; the current form is printed as C533 PT First Report, revised March 2022. The rates for non-contracted physiotherapists still list it as the assessment report, fee code RPT01.

Can someone other than a physiotherapist complete C533?

No. WCB-Alberta's information package for non-contracted physiotherapists says the physiotherapy reports cannot be completed by other disciplines. The report is signed by the physiotherapist, who also enters a WCB billing number and the name and address to whom the fee is payable.
