This two-page report is the first progress update a physiotherapist sends WCB-Alberta on a worker's treatment, printed as the PT Abbreviated Progress/Discharge Report. It focuses on return to work: whether a successful outcome is expected within the authorized physiotherapy episode, barriers to recovery, recommendations, the home exercise program, job-related abilities and estimated return-to-work dates.
The treating physiotherapist completes and signs the whole report; WCB-Alberta says physiotherapy reports cannot be completed by other disciplines.

If your copy of the form is a fillable PDF, upload it once and Vero Documents fills it from the visit note and the chart, ready for you to review and sign. Download, print, or email the finished form, with the patient’s consent.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Within 2 business days of the end of the second week of treatment under a standard authorization, or the end of the fourth week for fracture and surgery injuries, according to WCB-Alberta's information package for non-contracted physiotherapists. The comprehensive progress report follows at week 4, or week 8 for fracture and surgery.
Both carry form number C534. The abbreviated report is two pages on return-to-work prospects, barriers and job-related abilities. The comprehensive report adds examination findings such as range of motion, strength, pain, neurological and gait findings, the interventions used and patient engagement, and it is the report used to request a treatment extension.
