All 65 verified Alberta forms, from WCB-Alberta reports and auto injury forms to AISH and ADAP, driver medicals and Alberta Blue Cross special authorization.
Alberta has 65 verified forms in this library, all in English. WCB-Alberta accounts for 29, starting with the Physician first report (C050) and the Physician progress report (C151). Auto injury claims use the Superintendent of Insurance forms, such as the AB-1A disability claim and the AB-2 Treatment Plan for sprain, strain and WAD injuries. AISH and ADAP applications need the Disability Income Assistance Medical Report (DS2444B), and some drivers need the Medical Examination for Motor Vehicle Operators (TRANS3050).
WCB-Alberta requires electronic reporting for the C050 and C151, through myWCB or an accredited medical software vendor; paper copies can only be ordered. Alberta's Care-First auto insurance system starts January 1, 2027 with new forms. The AB forms and DS2444B are XFA files that open only in Adobe Reader. Driver medicals aren't insured by AHCIP, so the patient pays. Federal forms (T2201, CPP disability, EI) apply to Albertans too; see the federal hub.

No. WCB-Alberta requires electronic reporting for the Physician first report (C050), through myWCB Electronic Injury Reporting or an accredited medical software vendor, and paper copies can only be ordered. The C050 is a combined report and invoice. WCB-Alberta pays more for same-day and on-time reports than late ones, and the Act requires a report within two days of first attendance when disability beyond the accident day is likely.
Care-First starts January 1, 2027 with a new set of forms for health care practitioners, including the Initial Assessment and Care and Treatment Request, Program of Care reports, progress and concluding reports, and a Standard Invoice. No form numbers or downloadable templates were published when the library was checked, so confirm on the Government of Alberta's Care-First page.
The patient. The Medical Examination for Motor Vehicle Operators isn't insured by AHCIP, so the physician sets the fee and must tell the patient beforehand. Any Canadian-licensed physician or NP can complete Sections A and B. The form is valid for 6 months after completion, and the driver presents it at an Alberta registry agent.
The Disability Income Assistance Medical Report (DS2444B) is normally completed by the applicant's physician, and NPs, specialists and allied health may assist. Give it to the applicant to upload with the online application, or send it to the AISH and ADAP office by email, fax or mail. The applicant pays, but Income Support, AISH or ADAP may cover the cost with an expense approval letter.
