Care-First Treatment Concluding Report (Physical)

When customized physical treatment under Alberta's Care-First system ends, the treating practitioner completes this report for the auto insurer. It documents the outcomes of treatment and the recommended next steps, closing the sequence that began with the Initial Assessment and Care and Treatment Request (Physical). The Care-First forms apply to treatment from January 1, 2027.

Who completes it

The health care practitioner who provided the customized physical treatment completes the whole report.

Submitting and payment

Frequently asked questions

When is the Care-First Concluding Report (Physical) completed?

At the end of customized physical treatment. It documents treatment outcomes and the recommended next steps for the injured person, after the Initial Assessment and Care and Treatment Request (Physical) and any Care and Treatment Progress Forms (Physical).

Is the concluding report the same for Program of Care patients?

No. Injured people treated in the Care-First Program of Care finish with the Program of Care Phase 2 / Concluding Report, which documents functional status and outcome-measure progress at the end of the program. This report is for customized physical treatment.
