Frequently asked questions

When is the Care-First mental health concluding report used?

At the end of mental health treatment. It records the outcomes of treatment and the recommended next steps after the Initial Assessment and Care and Treatment Request (Mental Health) and any progress forms. The completed report goes to the insurer.

Does Care-First use one concluding report for all treatment?

No. Alberta lists separate concluding reports for mental health and for customized physical treatment, and the Program of Care ends with its own Program of Care Phase 2 / Concluding Report, which documents functional status and outcome-measure progress.
