Health care practitioners use this Alberta Care-First form to ask the auto insurer to approve a customized physical treatment plan for an injured person. Unlike the Initial Assessment and Care and Treatment Request (Physical), it does not establish a diagnosis. Care-First, with its own set of practitioner forms, starts January 1, 2027.
The health care practitioner recommending the customized physical treatment completes the whole form.

This form isn’t published as a fillable PDF. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

No. The standalone Care and Treatment Request (Physical) asks the insurer to approve a customized physical treatment plan but does not establish a diagnosis. The assessment and diagnosis are recorded on the Initial Assessment and Care and Treatment Request (Physical).
Care-First begins January 1, 2027, and Alberta's information for practitioners covers the treatment of injuries under the new model from that date. The existing AB claims forms, such as the AB-2 Treatment Plan, are prescribed for claims under the Diagnostic and Treatment Protocols Regulation.
