Under Alberta's Care-First auto insurance system, which starts January 1, 2027, a health care practitioner uses this form to record a physical assessment, a diagnosis and a recommended customized treatment plan for an injured person. The form goes to the insurer, which must approve the plan before customized treatment begins.
The treating health care practitioner who assesses the injured person completes the whole form, including the diagnosis and the recommended customized treatment plan.

Initial Assessment and Care and Treatment Request (Physical)
This form isn’t published as a fillable PDF. Vero Scribe writes the visit note it draws on, so the details are ready when you complete it.
A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms change without notice, so download the current version from the official page before you fill it in.

Before treatment begins. The insurer must approve the recommended customized treatment plan on the Initial Assessment and Care and Treatment Request (Physical) before the treatment it describes starts. Progress and concluding forms then track the treatment.
The Care and Treatment Progress Form (Physical) updates recovery progress, treatment response and changes in treatment need, and the Care and Treatment Concluding Report (Physical) documents outcomes and recommended next steps at the end of customized physical treatment. Mental health care has its own parallel forms.
